Admissions

 

At South Shore Hospital, your health matters to us. We try to provide the least invasive diagnostic and treatment options. Our Pre-Admission Testing Department staff are here to help. They will schedule an appointment that is convenient for you and arrange for the medical tests or procedures your physician orders to be done at that time.

The Admitting staff understand your health concerns while being admitted or registering for outpatient tests of surgery. We're dedicated to making this process as quick and efficient as possible for you. Our staff will help you prepare, keep you informed and put you at ease.

The Admitting Department never closes. They handle all non-emergency and emergency admissions to the hospital.

On the day of your PAT appointment, please arrive on time and bring your personal identification, your physician's authorization or directions for tests or surgery, the HMO referral form, and your insurance, HMO, Medicaid, or Medicare card.

Patient Representative

The Patient Representative is available Monday - Friday, excluding Holidays, from 7:00am - 3:00pm at (773) 356- 5398. After hours, weekends, and on holidays, the calls are answered by the Nursing Supervisor.

The Patient Representative is available to assist you and your family with explanations about our policies, procedures, and services. She is also available to assist in conflict resolutions should they arise during your stay.
